Shaping neighbourhood health through a household lens
This report argues that effective neighbourhood health services should look beyond individual patients and take account of the wider household and community circumstances that can influence health outcomes. It finds that factors such as housing, deprivation, caring responsibilities, social isolation and trauma can drive health needs and demand for services, yet are sometimes overlooked in traditional models of care. It advocates a shift from reactive care to a more preventive approach, where integrated neighbourhood teams use connected data and local partnerships to identify people at risk earlier and coordinate support across health, social care, local government and voluntary sector organisations.
