Dental forms changing

FP17DC forms will look different in the coming weeks. The updated forms are reflective of the changes made as part of the 2026 quality and payments reforms to the dental contract. The 2026 FP17DC (R8) replaces the 2025 version (R7). On page 1, find the new clinical and treatment capture fields, including a specific field for denture repair, and a new Care Pathway (1–3) section. Colleagues must not use the Care Pathway (1-3) section until this contract change is active. NHS England will communicate when this change has been made.

On page 2, the patient information now includes clearer guidance on care pathway eligibility and descriptions, paying for NHS treatment, recall and regular care, including typical recall intervals and advice if problems arise.
