Improving COPD Care with remote patient monitoring
Remote patient monitoring can support integrated care boards and primary care providers in delivering effective care for patients with chronic obstructive pulmonary disease (COPD). Clinitouch, the remote monitoring platform, provides comprehensive care for COPD patients, helping to reduce hospital admissions and improve outcomes.
With Clinitouch, you can monitor the patient’s condition, and the patient can access pulmonary rehabilitation through a single, streamlined platform. This integrated approach ensures COPD patients receive end-to-end care, improving their overall health.
Supporting patients and clinicians
Clinitouch keeps patients connected with their clinical teams, enabling early intervention and effective treatment. The platform helps prevent hospital admissions by ensuring patients get the help they need at the first signs of an exacerbation.
Features include:
- Remote monitoring: Tracking vital signs and symptoms from home.
- Proactive intervention: Detecting deterioration early and responding quickly.
- Pulmonary rehabilitation support: Providing a personalised exercise and education programme.
- Informed decision-making: Empowering clinicians with detailed data to support care planning and treatment decisions.
How does the platform work?
Patients are referred by a GP or hospital clinician. They use an app to answer COPD-related health questions and record vital signs like blood pressure and oxygen saturation at home. Clinicians can customise the health questions to ensure they gather the data they need.
The platform’s unique algorithms analyse the data, assigning risk scores and flagging concerns. The concern levels are displayed on the clinician dashboard, with a simple red, amber or green rating. This allows the clinical team to quickly see where care is required, enabling timely interventions to minimise exacerbations. high temperature, and signs of an exacerbation.

For pulmonary rehabilitation the patient is enrolled on to the programme for six weeks with in-person assessments at the beginning and end of the programme. The patient has access to Clinitouch to complete education and exercises while also having access to the respiratory team.
Proven clinical outcomes for COPD
Clinitouch has demonstrated positive clinical outcomes and is a tried-and-tested platform for COPD management. It has shown a 67.5% reduction in unscheduled COPD hospital admissions1and a 15.9% mean reduction in CAT score2. Additionally, it can save 2.73 bed days per patient3.
The platform is one of only six approved by NICE to support patient self-management of COPD.
A collaborative approach to COPD management
From early intervention to rehabilitation and ongoing support, Clinitouch integrates care on a single platform. By reducing hospital visits and supporting self-management, the platform aims to enhance the quality of life for COPD patients.
