A series of case studies showcasing integrated neighbourhood teams will be published. The first highlights Team Up Derbyshire, an integrated neighbourhood team approach in the Derby and Derbyshire integrated care system. It enabled a reduction of 2,300 category three ambulance call outs and reduced hospital stays by 1,400. They work across health and social care to see all people in a neighbourhood currently unable to leave home without support.
