Managing heart failure @home decreases GP and community nurse appointments
A report into the managing heart failure @home programme has found it is delivering value to patients, improving their quality of life and clinical wellbeing, while also likely to be reducing GP and nurse resource use in primary care. The evaluation report focused on a six-month pilot across 10 sites. The programme uses personalised care, remote support and monitoring, and increased integration of primary and secondary clinical services for heart failure patients who are not acutely unwell but who are being medically optimised or need optimisation.
