This publication sets out the model needed to make timely, clinically-led discharge routine – so all patients receive the right care, at the right time, no matter when or where they are treated. The model applies to hospital and community bedded care and addresses: in‑hospital discharge planning and delivery, daily ward-based practices within the control of hospital teams and standardising basic processes to reduce variation.
