Primary care transformation PCN hub

NHS England South East primary care transformation programme has published a blueprint document and other learning resources based on a regional pilot project to boost appointment capacity through federated delivery of services.  Using the enhanced access contract and other sustainable funding streams, the Folkestone, Hythe and Rural primary care network (PCN) created a new hub operating model which has added more than 2,500 additional appointments per month for its seven practices.  The resources describe the hub’s innovative technical solution which allows PCN staff to access patient records via a single log-on as well as its financial model, workforce structure, and clinical and data governance arrangements.  You can find a video here for an introduction to the project.
