Regional Learning from Incidents and Excellence – Event 7

You are invited to join us to learn from local successes, interesting, & challenging paediatric cases within Yorkshire & Humber.

Do you have a case to share?

Each of the 14 Hospital Trusts within the Yorkshire & Humber region (and in some cases each of the specialities within a trust) manage adverse clinical incidents separately, which are in turn investigated with actions initiated to prevent future similar occurrences.

The same is true for ‘Excellence’.

Outstanding clinical patient care, consistent and robust clinical effectiveness, and wider system successes are each also happening within the silos of individual hospitals.

No robust system exists for the benefit of shared learning from paediatric ‘Incidents’ and ‘Excellence’.

Learning within any one centre should be of benefit to all, across    our Paediatric Critical Care Network.

Goals:

• Promote regional collaboration and shared learning from incidents.
• Promote clinical excellence for children, young people, and their families.
• Engage multidisciplinary staff groups: i.e. Nurse/Medic/AHP, Governance & Service Improvement leads.
• Improved effectiveness, and reduced duplication.
• Shared governance, risk reduction, and patient safety ethos.
• Shared education resources.
• Bring together stakeholders not connected by existing systems
